　　06老年人健康管理項目及內容：對轄區內65歲以上常住居民，提供生活方式和健康狀況評估、體格檢查、輔助檢查、健康指導。體檢項目包括一般體格檢查與輔助檢查。輔助檢查項目包括血常規、尿常規、肝功、腎功、血糖、血脂、腹部B超、心電圖。體檢地點：桂陽社區衛生服務中心（南陽轉盤紅綠燈旁或春花社區消防大隊旁）；體檢時間：周一到周五上午8：00—10:00，節假日不體檢；注意事項：早上空腹，攜帶身份證或醫保卡。

　　07慢性病患者健康管理項目及內容：轄區內35歲以上常住居民中原發性高血壓和2型糖尿病患者，檢查發現、隨訪評估和分類干預、健康體檢。在我中心建立了高血壓或糖尿病健康檔案，簽約了家庭醫生，家庭醫生會定期提供慢性病患者健康管理服務；患者會在醫生的指導下建立健康的生活方式，合理使用降血糖或降血壓的藥物，將血糖、血壓控制在理想水平，最大限度地減少糖尿病或高血壓給健康帶來的危害；在管理過程，醫生會及時發現其他健康問題，及時調整治療方案。高血壓、糖尿病患者可在桂陽社區衛生服務中心住院部或是南陽綜合門診辦理高血壓或糖尿病特病。需攜帶身份證和醫保卡。

　　10中醫藥健康管理項目及內容：對轄區內65歲及以上常住居民和0-36個月常住兒童，提供老年人中醫體質辨識、兒童中醫調養。
